Rural counties, which have the highest rates of unemployment and poverty within the state, stand to lose the most. Tulare County, located in the San Joaquin Valley, leads with the highest Medi-Cal enrollment at 55 percent of its residents, with Merced close behind at 51.5 percent, and Fresno at 49.9 percent.
